Important Medical Information When Every Second Matters

The Vial of Life program helps emergency responders quickly find important medical information when a patient may be unable to communicate.

Participants complete a medical information form, place it inside the provided vial, and store it in the freezer. A magnet on the refrigerator and a sticker near the front entrance alert EMS responders that the information is available.

Fold the completed medical information form and place it inside the provided Vial of Life container.

Keep the vial in the freezer so responders know exactly where to look during an emergency.

Put the Vial of Life magnet on the refrigerator to alert emergency responders that medical information is available.

Place the Entrance Sticker

Place the sticker near the front entrance so responders can quickly identify that the home participates in the program.

Call 911 when someone may be experiencing a serious or life-threatening emergency, including chest pain, signs of stroke, severe difficulty breathing, loss of consciousness, uncontrolled bleeding, seizure activity, or a serious injury. When in doubt, call.

Provide the location of the emergency, a callback number, what happened, the patient’s condition, and any immediate hazards. Answer the dispatcher’s questions as clearly as possible and do not hang up until you are instructed to do so.

Follow the dispatcher’s instructions and remain with the patient when it is safe to do so. Turn on outside lights, unlock the entrance, secure pets, and send someone to guide responders to the patient when possible.
