Participants complete a medical information form, place it inside the provided vial, and store it in the freezer. A magnet on the refrigerator and a sticker near the front entrance alert EMS responders that the information is available.
Update the form whenever medications, medical conditions, emergency contacts, or other important information changes.
Record current medical conditions, medications, allergies, emergency contacts, and other important health information.
Fold the completed medical information form and place it inside the provided Vial of Life container.
Keep the vial in the freezer so responders know exactly where to look during an emergency.
Put the Vial of Life magnet on the refrigerator to alert emergency responders that medical information is available.
Place the sticker near the front entrance so responders can quickly identify that the home participates in the program.
