How to Request Your Medical Records
To request a copy of your medical records, complete the authorization form and submit it to the Health Information Management department. Providing complete and accurate information can help prevent delays.
Complete the authorization form
Fill out the Authorization for Release of Medical Information form and provide the patient and contact information requested.
Identify the records you need
Tell us which records should be released, the dates of service and where the requested information should be sent.
Sign and submit your request
Sign the completed form and submit it to Health Information Management using one of the delivery options provided below.

Authorization & Access
Who Can Request Medical Records
Patients may request their own medical records or authorize another person or organization to receive them. Additional documentation may be required when a request is made by a representative.
The patient
Patients may request copies of their own medical records by completing and signing the authorization form.
A parent or legal guardian
A parent or legal guardian may request records when legally authorized to act on behalf of a minor or dependent patient.
An authorized representative
A person with legal authority to act for the patient may request records with the appropriate supporting documentation.
An approved third party
A provider, attorney, insurance company or another organization may receive records when properly authorized by the patient.

How do I request a copy of my medical records?
Complete and sign the Authorization for Release of Medical Information form. Identify the records you need, include the applicable dates of service and tell us where the records should be sent.
Can I request only part of my medical record?
Yes. You may identify the specific records or dates of service you need on the authorization form. Providing detailed information helps Health Information Management locate the correct records.
Can my records be sent to another provider?
Yes. On the authorization form, provide the name and contact information of the provider or facility that should receive the records.
Can someone else request records for me?
A parent, legal guardian, personal representative or another authorized person may be able to request records on a patient’s behalf. Health Information Management may require documentation showing that the person is authorized to act for the patient.
How long will my request take?
Processing time may depend on the records requested and whether additional information is needed. For questions about the status of a request, call Health Information Management at 432-607-3203.

Can I email my medical records request?
