Now located in the Medical Office Building, the clinic gives patients coordinated access to laboratory services, imaging, therapy, Express Care, and specialty care within one connected setting.

Our team coordinates access to laboratory services, imaging, therapy, specialty care, and Express Care within the Medical Office Building.
Chronic Care Management
Managing a long-term condition takes more than an occasional appointment. Our in-house Chronic Care Management team works alongside your primary care provider to help you understand your care plan, stay on track, and reduce the risk of complications.
Through regular one-on-one support, patients receive practical guidance for managing their health at home and making informed decisions between visits.
Diabetes Education & Support
The MCHD Diabetes Learning and Support Center provides individualized Diabetes Self-Management Education and Support for people with Type 1, Type 2, and gestational diabetes. Education is provided one-on-one, giving patients time to ask questions, build practical skills, and focus on the challenges that matter most to them.
The MCHD Diabetes Learning and Support Center is accredited through the ADCES Diabetes Education Accreditation Program (DEAP), recognizing that our program meets national standards for diabetes self-management education and support.
