Martin County Hospital District is proud to recognize Jaclyn Guzman, LVN, as the June Employee of the Month for her attention to detail, clinical awareness, and commitment to patient safety.
Martin County Hospital District recognizes Jaclyn Guzman as the June Employee of the Month after catching a serious insulin dosing error during medication reconciliation.
Jaclyn was recognized after identifying a serious medication concern while completing medication reconciliation for a patient in the clinic.
While reviewing the patient’s insulin regimen, she noticed that the documented dosing did not match what the patient reported taking. Jaclyn investigated further and discovered that the prescription had been written incorrectly, resulting in the patient taking an excessive amount of insulin.
Her careful review and willingness to investigate the discrepancy helped prevent what could have become a life-threatening situation.
Catching What Others Might Miss
Medication reconciliation is one of those tasks that can look routine on paper but carries enormous responsibility.
Jaclyn’s careful review of the patient’s insulin use revealed that something was not adding up. Rather than accepting the information at face value, she took the time to investigate further and identify the underlying problem.
That attention to detail helped ensure the patient received the correct information and avoided continued exposure to an unsafe dose.
Patient Safety Starts with Vigilance
Safe patient care depends on healthcare professionals being willing to question inconsistencies and speak up when something does not seem right.
Jaclyn’s actions demonstrate the importance of vigilance, accountability, and careful communication in every patient interaction. Her work helped protect the patient from a potentially serious outcome and reflects the responsibility MCHD employees bring to patient care every day.
